BRAZILIAN JIU-JITSU LIABILITY WAIVER AND RELEASE OF CLAIMS

This Liability Waiver and Release of Claims (“Agreement”) is entered into on this {sign_date} , by and between {name} and Alliance Easley, including its owners, instructors, employees, agents, representatives, and affiliates.

1. Acknowledgment of Risk

Participant understands that Brazilian Jiu-Jitsu and martial arts training involve physical contact and inherent risks. These risks may include, but are not limited to:

• Joint locks and submission holds
• Choking techniques
• Throws and takedowns
• Slips, falls, and collisions
• Strains, sprains, fractures, and dislocations
• Cuts, bruises, or other bodily injuries

Participant acknowledges that injuries may occur during instruction, drilling, sparring (rolling), or any other activities conducted at the gym.

Participant voluntarily accepts and assumes all risks associated with entering the premises and participating in Brazilian Jiu-Jitsu training or related activities.

2. Release of Liability

To the fullest extent permitted by law, Participant hereby releases, waives, and discharges the Gym/Host, including its owners, instructors, employees, agents, and affiliates, from any and all liability, claims, demands, damages, costs, expenses, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur while participating in activities or while present on the premises.

3. Indemnification

Participant agrees to indemnify and hold harmless the Gym/Host from any loss, liability, damage, or costs (including reasonable attorney’s fees) arising from Participant’s presence on the premises or participation in activities.

4. Responsibility for Personal Property

Participant acknowledges that the Gym/Host is not responsible for lost, stolen, or damaged personal property brought onto the premises.

5. Compliance with Rules

Participant agrees to follow all gym rules, safety guidelines, and instructions provided by instructors or staff. Failure to comply may result in removal from the premises.

6. Medical Responsibility

Participant affirms they are physically capable of participating in martial arts training and understands that the Gym/Host does not provide medical insurance coverage. Participant is responsible for any medical expenses resulting from injury.

7. Health and Hygiene

Participant agrees not to participate in training if they have any contagious illness, skin infection, or open wounds that may pose a risk to others.

8. Electronic Signature Consent

Participant agrees that signing this form electronically constitutes a legally binding signature. Participant acknowledges that their electronic signature has the same legal effect as a handwritten signature.

9. Communication Consent

Participant agrees that the Gym/Host may contact them via phone, email, or text message regarding class schedules, trial sessions, memberships, promotions, and other gym-related communications.

10. Governing Law

This Agreement shall be governed by and interpreted in accordance with the laws of the State of South Carolina .

11. Acknowledgment

By signing below, Participant acknowledges that they have read this Agreement, fully understand its terms, and voluntarily agree to be legally bound by it.
